Our core values—Compassion, Passion for Results, Collaboration, Accountability, Integrity, and Conscientiousness—guide us in creating meaningful, lasting impacts in the communities we serve.  Salary: $21 - 25 hr Schedule: Mon - Fri 8-4pm (Weekends when needed) Job Summary:   The Tenancy Support Specialist provides pre-tenancy and tenancy support services to participants enrolled in the H2O program. This role focuses on assisting individuals designated as Seriously Mentally Ill (SMI) in obtaining and maintaining stable housing while connecting them with community resources and support systems. Responsibilities: 

Provide pre-tenancy and ongoing tenancy support to a caseload of approximately fifteen (15) participants in accordance with H2O program requirements. Maintain a minimum of four participant contacts per week, including at least one in-person visit, as required by AHCCCS H2O standards. In-person meetings may take place in the participant’s home, clinic, shelter, or other community settings. Ensure timely and accurate completion of referrals, case notes, assessments, and required reports in accordance with billing and compliance standards. Utilize a personal or agency vehicle to transport participants to meetings and appointments related to housing searches and supportive services. Maintain ongoing coordination with community resources and housing stabilization services for Seriously Mentally Ill (SMI) participants referred by Solari. Collaborate with participants’ Health Homes, Health Plans, Network Administrators (such as Solari and Banner), and other housing partners to ensure effective delivery of H2O support services. Advocate for and assist participants in finding affordable housing and accessing appropriate housing subsidy programs. Help participants gather necessary documentation for resource connections. Maintain compliance with all initial and ongoing training and regulatory requirements set by the agency, system partners, and AHCCCS. Navigate and utilize multiple platforms and systems for documentation, including HMIS, Unite Us, SharePoint, and other databases required by the H2O initiative. Assist participants in implementing Housing Care Plans that overcome barriers, boost income, sustain permanent housing, and improve mental and physical health. Provide advocacy, case management, benefits assistance, and linkage to mental health and substance use services. Collaborate with the participant’s Health Home treatment team to access mental health, substance use, and other supportive services as needed. Provide services to participants in independent living skills, employment assistance, benefits acquisition, and other supports as needed to help maintain stable housing. Assist participants with locating, applying for, and moving into new housing units. Provide support with utility setup and offer donations or start-up items as needed. Foster and promote an environment that is strength-based, trauma-informed, and culturally competent. Maintain an accurate tracking system for the assigned caseload and provide reports to the Program Manager or designee as required for contractual reporting. Prepare case-related reports, documenting outcomes, successes, and challenges. Attend and actively participate in collaborative meetings, regular supervisory check-ins, staff meetings, and required clinical trainings or oversight sessions. Engage in networking with other agencies, coalitions, and community stakeholders to enhance service coordination. Ensure ongoing compliance with all H2O program training requirements. Additional responsibilities as assigned. 

Qualifications:  Education: 

High school diploma, GED, or equivalent required Associate or Bachelor's degree, preferred

Experience: 

Minimum of six (6) months-1 year of behavioral health experience required Minimum of six (6) months-1 year supporting diverse homeless populations, including families, individuals, young adults, and veterans experiencing homelessness required. Demonstrated ability to manage a caseload effectively and provide individualized support to participants.

Working Conditions/Physical Requirements: 

Primarily office-based work with travel to client homes, meetings, and site visits. Ability to sit for extended periods while working on a computer. Occasional lifting of up to 25 lbs. for materials or office supplies. Exposure to typical office equipment, such as computers, phones, and printers. Ability to manage multiple tasks and cases Must be able to maintain confidentiality and professionalism in all client and team interactions.

Must Be At Least 25 Years of Age Class One Fingerprint Clearance Card DCS/DES Central Registry Background Check Drug Screening TB Test (Depending on Program) CPR/First Aid Certification Therapeutic Options Training (Depending on Program) Valid Unrestricted Driver’s License Current Auto Liability Insurance 39 Month Motor Vehicle Record Adhere to all organizational policies, including the Code of Conduct, professional standards, and relevant regulations.

Mandated Reporter Requirements: 

As a mandated reporter under Arizona statute A.R.S. § 13-3620, you are required to immediately report any suspected abuse or neglect of a minor to your Supervisor, Manager, Director, Clinical Director, or Chief Officer, who will assist in making the appropriate report to authorities.  Reports should be directed to either the local law enforcement agency or child protective services (1-888-SOS-CHILD). Abuse suspected of involving a family member or legal guardian should be reported to child protective services, while other cases should be reported to law enforcement.
