The Role Are you an MSW who believes that hospice care is about more than just the final days? It’s about dignity, family connection, and emotional peace. We are looking for a compassionate Hospice Social Worker to join our team. In this role, you won't just manage cases; you’ll hold hands, navigate complex emotions, and provide the "soft landing" every family needs during their hardest moments. What you’ll do:

Provide emotional support and crisis intervention. Assist families with end-of-life planning and resource navigation. Be the bridge between the clinical team and the heart of the home.

Continuum Care Hospice is looking to hire an experienced and dedicated social worker to work with our team of professionals.
