Position Summary: The Prevention Supervisor is primarily responsible for providing case management and counseling services to identified youth and families. Responsibilities include advocacy, referral and linkage, group, individual and family counseling and crisis intervention. Essential Job Functions:
Utilizing the task centered case management model, implements and coordinates services to children and their family's having trouble in the home and/or in school, aimed at preventing placement of children outside the home and/or at improving academic performance. Provides intervention and support to families in crisis intended to keep the family unit intact. Assesses children and families for risk, develops and implements service plans to address risk factors, drafts petitions and testifies in Family Court as necessary. Provides Foster Care Services and locates placements when needed. Completes all required documentation, in timelines in accordance with program standards. Advocates on behalf of children and their families to ensure adequacy of services (i.e., educational, legal and community) by providing linkage and coordination with the county providers and agencies.
Qualifications: Bachelor’s and two (2) years’ experience working in a school- and/or home-based setting providing services to individuals and/or groups. Experience working with families and their needs. Must have valid New York State driver’s license and access to reliable transportation. Required Skills and Abilities: Strong oral, written and public speaking communication skills. Must also possess strong problem solving, decision making and conflict resolution skills and exhibit a high level of self-confidence and responsibility. Must be able to work with complex systems (ex. Probation, Family Court, Foster Care) and have knowledge and/or experience with both the child welfare and juvenile justice systems. Ability to work independently and with flexibility in days and hours of work. Experience doing home-based work in a rural setting preferred. Must have access to reliable transportation and have a valid driver’s license.
