We’re committed to building a diverse workforce that values contributions of all team members and work to recognize cultural beliefs, values, traditions, language preferences, and health practices of the communities that we serve and how they apply to provision of positive health outcomes.     Position Summary    Under the direct supervision of the Prevention Supervisor, the Caseworker is primarily responsible for providing case management and counseling services to identified youth and families.  Responsibilities include advocacy, referral and linkage, group, individual and family counseling and crisis intervention.   Essential Job Functions  

Utilizing the task centered case management model, implements and coordinates services to children and their family's having trouble in the home and/or in school, aimed at preventing placement of children outside the home and/or at improving academic performance.   Provides intervention and support to families in crisis intended to keep the family unit intact. Completes all required documentation, in timelines in accordance with program standards.

  Duties and Responsibilities  

Assesses children and families for risk, develops and implements service plans to address risk factors, drafts petitions and testifies in Family Court as necessary.  Provides Foster Care Services and locates placements when needed.

 

Advocates on behalf of children and their families to ensure adequacy of services (i.e., educational, legal and community) by providing linkage and coordination with the county providers and agencies.  

 

Processes and reviews intake of all referrals to program.  

 

Provides services in a manner which is culturally competent. Empowers clients by using clients’ cultural strengths and informal support networks in service delivery.

 

Maintains effective professional relationships with other service providers and community organizations in the interest of clients’ and program goals.

Identifies and reports on Untoward Incidents as required.

 

Carries out Agency policies and procedures.

 

Completes all necessary written reports, statistics, case files and summaries in a timely manner and in accordance with program and agency standards.  

 

Attends and participates in individual supervisory conferences, staff meetings, training sessions and professional development opportunities as required.  Maintains required program certifications.

 

Provides door-to-door transportation as required.  Ensures safety and welfare of passengers during loading, unloading and transit.  Make use of personal vehicle for program transportation as required.  

 

Handles emergency situations which may require flexibility of time and area of assignment.

 

Performs other duties, responsibilities and related tasks as assigned.

  Qualifications   Bachelor’s and two (2) years’ experience working in a school- and/or home-based setting providing services to individuals and/or groups.  Experience working with families and their needs.  Must have valid New York State driver’s license and access to reliable transportation.     Required Skills and Abilities   Strong oral, written and public speaking communication skills.  Must also possess strong problem solving, decision making and conflict resolution skills and exhibit a high level of self-confidence and responsibility.  Must be able to work with complex systems (ex. Probation, Family Court, Foster Care) and have knowledge and/or experience with both the child welfare and juvenile justice systems.  Ability to work independently and with flexibility in days and hours of work.  Experience doing home-based work in a rural setting preferred.  Must have access to reliable transportation and have a valid driver’s license.    Salary: $23.08 Per Hour   #INDFS
