Kvalifikační předpoklady- odborná způsobilost sociálního pracovníka dle zákona č. 108/2006 Sb., o sociálních službách (min. vyšší odborné vzdělání sociálního směru) Požadujeme- práce s PC (MS Office, program Cygnus výhodou)- znalost individuálního plánování výhodou- řidičský průkaz sk. B – aktivní řidič- trestní bezúhonnost – prokazována výpisem z rejstříku trestů- spolehlivost, pečlivost a zodpovědný přístup k práci- ochotu vzdělávat se- schopnost týmové spolupráce, komunikační dovednosti- schopnost podporovat a pomáhat k soběstačnosti, pozitivní přístup ke klientůmNáplň práce:- provádí sociální šetření- provádí jednání se zájemci o sociální službu- vypracovává a aktualizuje vymezení schopností člověka zvládat základní životní potřeby- dává podnět k přiznání nebo přehodnocení příspěvku na péči- připravuje podklady pro uzavření smlouvy a jejich příloh s klientem - podepisuje s klientem smlouvu o průběhu sociální služby a jejich příloh při nástupu klienta do zařízení - spolupracuje s opatrovníkem- zabezpečuje základní sociální agendu, včetně vedení sociální dokumentace- poskytuje základní sociální poradenství - zprostředkovává klientům základní sociální a společenské kontakty- dohlíží na dodržování vnitřních standardů a platné legislativyNabízíme:- odpovídající platové ohodnocení, plat na plný pracovní úvazek 29.740,- Kč až 43.180,- Kč (dle nařízení vlády č. 341/2017 Sb. podle dosažené kvalifikace a praxe) + osobní ohodnocení + odměny- 5 týdnů dovolené- firemní benefity:o závodní stravování o pravidelný příspěvek na penzijní připojištění o zaměstnanecké masáže- kolegiální a přátelskou atmosféru - zodpovědnou činnost u důvěryhodného zaměstnavatele.Pracovní doba 40 hodin týdně nerovnoměrně rozvržená (jednosměnný provoz).Nástup dle domluvy.
