Manages a complex participant caseload, promotes consistent application of program requirements, serves as a peer resource, and supports supervisory and team initiatives that strengthen service quality, compliance, and participant outcomes within the Wisconsin IRIS program.

Essential Job Duties

  • Manages a complex IRIS participant caseload and delivers consultation, planning, monitoring, and follow-up activities in accordance with program requirements, participant needs, and established service standards.
  • Conducts required participant contacts, including in-person visits and annual home visits, and remains responsive to participant requests in support of the self-directed nature of the IRIS program.
  • Delivers and oversees orientation to new participants regarding program rights, responsibilities, enrollment documentation, and stewardship of public funds.
  • Facilitates person-centered planning by assessing participant goals, relationships, natural supports, community connections, employment interests, backup plans, and awareness of available program options.
  • Guides and partners with participants in identifying desired outcomes and monitors plans on an ongoing basis to ensure services remain aligned with participant goals, budgets, and program requirements established by the Wisconsin Department of Health Services.
  • Ensures accurate and timely documentation of participant contacts, planning activities, plan changes, and required system updates within established turnaround times.
  • Guides participants in understanding budget reports, amendment requests, one-time expense processes, provider payment issues, and other administrative requirements to support compliant budget management.
  • Researches and shares community resources, natural supports, and service options that advance participant outcomes and reduce barriers to successful plan implementation.
  • Identifies, escalates, and helps resolve participant, service, documentation, or compliance issues by applying sound judgment, problem-solving skills, and knowledge of program requirements.
  • Serves as a peer resource to other IRIS Consultants by sharing best practices, reinforcing consistent service delivery and documentation practices, and supporting onboarding and mentoring of less experienced staff.
  • Supports supervisors and operational leaders through participation in team initiatives, committees, workgroups, audits, workflow support, and other assigned activities that improve team performance and program effectiveness.
  • Promotes compliance with company policies, confidentiality standards, and applicable program requirements while maintaining effective working relationships with participants, colleagues, and internal and external partners.
  • Attends and actively participates in team meetings, case consultation, training, and continuous improvement efforts to support consistent, high-quality IRIS Consulting services.
  • Local travel required.

Required Qualifications

  • At least 4 years of experience related to the delivery of social services to target populations, including older adults and individuals with intellectual and/or physical disabilities, or equivalent combination of relevant education and experience.
  • Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field (or 4 years of commensurate experience if no degree).
  • Valid and unrestricted driver’s license, reliable transportation, and adequate auto insurance for job related travel requirements unless otherwise required by law.
  • Knowledge of long-term care programs and familiarity with principles of self-determination, person-centered planning, and community-based service coordination.
  • Demonstrated ability to work independently with minimal supervision while managing multiple priorities, deadlines, and participant needs.
  • Strong problem-solving, critical thinking, and decision-making skills, including the ability to identify risks, evaluate options, and recommend appropriate next steps.
  • Strong verbal and written communication skills, with the ability to build effective relationships with participants, families, providers, and internal partners.
  • Ability to adapt communication and work style to a variety of settings, diverse populations, personalities, and sensitive personal situations.
  • Resourceful approach to researching community resources, resolving issues, and supporting participant access to services and natural supports.
  • Proficiency with Microsoft Office applications and other technology used for documentation, communication, reporting, and data entry in online systems and databases.
  • Demonstrated ability to model consistency, provide peer support, and reinforce established policies, procedures, and service expectations.

Preferred Qualifications

  • Experience supporting onboarding, mentoring, training, or peer development activities.
  • Demonstrated experience participating in committees, special projects, quality initiatives, or process improvement efforts.
  • Strong working knowledge of IRIS program operations, documentation expectations, and company policies and procedures.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
