Pay: $70,000 – $85,000 per year base, plus uncapped commission

Job Type: Full-time | Field-based

At Moments Hospice, we believe every patient deserves comfort, dignity, and compassionate care in the final chapter of life. As a Hospice Care Consultant, you'll help make sure patients and families learn about hospice early enough to get the full benefit of it. You'll be the face of Moments Hospice in your territory, building trusted relationships with the healthcare professionals who refer patients to us.

You'll have clear goals, structured training, and a team invested in your success. Every relationship you build helps another family get the care they deserve.

Responsibilities:

  • Generate referrals for new patient admissions, the core focus of this role
  • Visit hospitals, skilled nursing facilities, assisted living communities, home health agencies, clinics, and physician offices to build and grow referral relationships
  • Educate healthcare professionals, patients, and families about hospice services and the value of early referrals
  • Plan your territory, make outreach calls, and give presentations to current and potential referral sources
  • Represent Moments Hospice at community events, professional associations, and educational in-services
  • Stay current on market trends and competitors, and build outreach plans that drive referral growth
  • Lead contract negotiations with facilities, insurance companies, and managed care providers
  • Partner with our clinical team to develop educational programs and address referral source concerns
  • Track your outreach and results, and work toward referral and admission goals

Responsibilities:

  • 1 year of outside B2B sales experience; experience in healthcare sales, hospice, home health, medical device, or clinical liaison roles preferred
  • Bachelor's degree preferred
  • Strong communication, presentation, and relationship-building skills
  • Self-motivated, organized, and able to manage a territory independently
  • Compassion for patients and families facing end-of-life decisions
