ASEANA HEALTHCARE PTE LTD is looking for aresponsible, organised and caring Care Coordinator to join our growinghealthcare team. 📋 Key Responsibilities

  • Coordinate and manage ICS referrals from hospitals
  • Contact clients and families to arrange the start of care services
  • Coordinate Nursing Aide/Caregiver assignments and schedules
  • Monitor daily attendance and ongoing client services
  • Arrange replacement staff when necessary
  • Communicate regularly with clients, families, hospitals and the ICS team
  • Monitor clients' service periods and upcoming discharge dates

👤 Who We're Looking For

  • Diploma or relevant experience in Healthcare, Nursing, Community Care, Social Services or Healthcare Administration
  • Experience in healthcare/home care coordination is an advantage
  • Good communication and interpersonal skills
  • Well-organised and able to manage multiple cases
  • Comfortable coordinating staff schedules and handling last-minute changes
  • Good computer and administrative skills
