The posting
- Support the planning and scheduling of home care services according to clients’ care plans, service requirements and staff availability.
- Maintain accurate service rosters and coordinate timely adjustments for leave, absences, emergencies and other operational changes.
- Facilitate effective coordination between care coordinators, nurses, care staff, clients and families to support continuity of care.
- Prepare and maintain service documentation, including client records, attendance records, care logs, consent forms and subvention-related information.
- Ensure timely and accurate data entry into CRM, case management and reporting systems.
- Prepare operational, service utilisation and other reports for internal monitoring and funder requirements.
- Maintain proper records and documentation in accordance with organisational policies and PDPA requirements.
- Coordinate operational logistics, including uniforms, equipment, consumables and other supplies required by care staff.
- Monitor inventory levels and support procurement and stock management for home care operations.
- Assist with timesheet verification, service billing, claims and other administrative processes to support accurate and timely payment.
- Liaise with finance, administration and vendors on invoices, reimbursements and payments where required.
- Act as a key administrative contact for care staff, clients and family members, handling enquiries professionally and escalating matters appropriately.
- Coordinate with external agencies, service partners and other stakeholders involved in service delivery.
- Support service quality monitoring by gathering feedback, following up on concerns and assisting with service improvement initiatives.
- Assist with audits, inspections, service evaluations and other quality or compliance exercises.
- Contribute to process improvements that enhance operational efficiency, service coordination and client experience.
- Provide administrative support to other community care programmes and undertake other duties or projects assigned by the HOD/Supervisor.
Job Requirements
- Diploma in Healthcare Management/Business administration or relevant
- Min 2 year of relevant finance/accounting experience
- Have experience with Microsoft Office
- Working Hour: Weekdays, 9am to 6pm
