A leading healthcare and eldercare organisation is seeking a Medical Social Worker and a Senior Medical Social Worker to join its growing nursing home team.

Job Responsibilities:

  • Screen referrals and assess suitability for admission with nursing teams and stakeholders
  • Supervise and support the case management team in completing pre-admission processes
  • Conduct bio-psychosocial-spiritual assessments and evaluate financial status to develop care plans with the multi-disciplinary team
  • Provide counselling and psychosocial interventions to residents and their families
  • Facilitate family conferences on care planning, including discharge, advance care planning (ACP), lasting power of attorney and deputyship
  • Support residents and families in adapting to the new living environment and provide caregiver support resources
  • Assess eligibility for subsidies and manage financial assistance cases (e.g. Medifund, MFEC)
  • Support discharge planning, including referrals to other care options and assistive equipment
  • Keep resident records and social reports updated, and work with Finance on accounts receivable when required
  • Manage and supervise case management team members

Job Requirements:

  • Degree or Postgraduate Diploma in Social Work
  • Registered with the Singapore Association of Social Workers (SASW)
  • Minimum 2 years of relevant experience in intermediate and long-term care settings
