The posting

  • Assist in developing individualised care and support plans for residents according to their assessed needs and the Home’s established procedures.
  • Support the implementation of care plans, monitor residents’ progress and report relevant changes or concerns to the appropriate personnel.
  • Organise and implement suitable programmes and activities to promote residents’ well-being, engagement and quality of life.
  • Conduct monthly resident interviews and maintain accurate records, including six-monthly review reports, for approximately 50 residents. We regret only shortlisted candidates will be contacted.
