Responsibilities
- Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.
- Provide intervention such as psychoeducation, supportive counselling and service linkages.
- Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
- Provide case recordings and relevant documentations.
- Participate in team meetings and case conferences as required.
- Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
- Plan and facilitate Caregiver Support Group sessions.
- Plan, support and facilitate outreach events and talks.
- Any ad-hoc tasks as and when required.
Requirements
- Degree or post-graduate diploma in social work, psychology or counselling. Relevant degree in allied health profession will be considered.
- Preferably 2 years of healthcare/community care clinical experience.
- Good oral, written and interpersonal communications skills.
- Able to work on occasional Saturdays and weekday evenings.
- Prior working experience in dementia care will be an added advantage.
