The posting
- Conduct case assessment and formulate Individualized Care Plan for persons with dementia and their carers.
- Plan and facilitate group sessions/ activities for carers and/or persons with dementia.
- Provide intervention such as psychoeducation, supportive counselling and service linkages.
- Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
- Provide timely and accurate case recordings and relevant documentations.
- Participatein team meetings and case conferences as required.
- Assist promptly in the preparation of data and statistics for programme evaluation and reporting purposes.
- Explore new avenues of collaboration with community partners and stakeholders.
- Support in outreach events and talks.
- Any ad-hoc tasks as and when required.
Requirements:
- Degree in Social work and Psychology, or any relevant discipline, with a minimum of 2 years of experience in community engagement and/or case management.
- Ability to conduct Advance Care Planning conversation or prior experience working with persons with dementia or carers will be an added advantage.
- Requires work on scheduled Saturdays and occasional weekday evenings.
- Good oral, written and interpersonal communication skills.
- Ability to facilitate group sessions will be an added advantage.
- Self-motivated with a high level of initiative.
- Resourceful and adaptable.
- Strong attention to detail.
- Good organizing and execution skills.
