The posting

As the primary contact for clients, the Registered Staff Nurse manages the treatment and recovery of clients by administering preventive, curative and rehabilitative care with the Home Personal Care team.

  • Conduct home visits and comprehensive nursing assessments for clients.
  • Assess client's medical conditions, nursing needs, functional abilities and home environment.
  • Provide nursing procedures within the nurses competency and authorised scope of practice; including wound care, catheter care and nasogastric tube care, where indicated.
  • Monitor vital signs, medication compliance and changes in client's health conditions.
  • Maintain accurate and timely nursing documentation, care plans, visit records and incident reports.
  • Support discharge planning, continuity of care and follow up for client transitioning from hospital to home.

Nursing Diploma and Degree graduates who are registered with the Singapore Nursing Board (SNB) may work as a Staff Nurse.
