Pour le compte d'un de nos clients, nous sommes à la recherche d'un Assistant social (H/F).

Vos futures responsabilités ##

  • Vous conduirez des entretiens individuels et de réseau pour analyser les situations des patients et identifier leurs difficultés sociales et financières ainsi que leurs ressources.
  • Vous élaborerez des solutions adaptées et accompagnerez les patients dans leurs choix, avec un accompagnement individualisé.
  • Vous collaborerez étroitement avec l'équipe pluridisciplinaire et les partenaires externes pour garantir un accompagnement global et cohérent.
  • Vous participerez activement au maintien à domicile des patients, en proposant des interventions adaptées à leurs besoins spécifiques.
  • Vous veillerez à établir une communication claire et bienveillante avec les patients, les familles et vos collègues pour favoriser une prise en soins humaine et globale.

Votre profil ##

  • Vous êtes titulaire d'un Bachelor HES en travail social à orientation service social ou d'un diplôme jugé équivalent.
  • Vous avez une première expérience réussie dans le canton de Vaud, idéalement en soins à domicile.
  • Vous faites preuve d'un excellent sens relationnel, ce qui vous permet de créer rapidement une relation de confiance avec les patients et les partenaires.
  • Vous êtes organisé, autonome et sensible à la prise en charge sociale et financière des patients.
  • Vous êtes titulaire d'un permis de conduire et disposez d'un véhicule pour vos déplacements professionnels.
