Depending on departmental needs, you may occasionally provide physical therapy services in skilled nursing facilities (SNF) and home health settings. The rehabilitation department also serves local schoolchildren, offering exposure to a diverse patient population and a wide range of clinical needs.

This opportunity is ideal for a Physical Therapist who values clinical variety, strong teamwork, and a supportive rural healthcare environment. The facility offers a dedicated rehabilitation team with extensive experience and high employee engagement, recently achieving the 97th percentile in employee engagement on a Press Ganey survey. With competitive compensation, a sign-on bonus of up to $10,000, flexible scheduling options, and limited weekend and holiday requirements, this position offers an opportunity to make a meaningful impact in a close-knit community.

Key Responsibilities

  • Evaluate and treat patients requiring physical therapy services, primarily in an outpatient rehabilitation setting, with additional inpatient coverage.
  • Conduct comprehensive physical therapy assessments, including evaluations of mobility, strength, balance, range of motion, functional limitations, and rehabilitation needs.
  • Develop and implement individualized treatment plans based on patient diagnoses, functional goals, and clinical findings.
  • Provide evidence-based therapeutic interventions to improve mobility, physical function, strength, endurance, balance, and overall independence.
  • Manage patients with orthopedic, neurological, musculoskeletal, post-surgical, and other conditions requiring physical rehabilitation.
  • Provide inpatient physical therapy services to support mobility, functional recovery, and discharge planning.
  • Occasionally provide coverage in skilled nursing facilities (SNF) and home health settings, depending on patient needs and departmental requirements.
  • Support rehabilitation services for a diverse patient population, including adults, older adults, and pediatric patients when assigned and appropriately qualified.
  • Monitor patient progress, reassess functional outcomes, and modify treatment plans to support continued improvement.
  • Educate patients and caregivers regarding therapeutic exercises, mobility techniques, injury prevention, safety precautions, and home exercise programs.
  • Collaborate with physicians, occupational therapists, nursing staff, case managers, and other healthcare professionals to coordinate comprehensive rehabilitation care.
  • Maintain accurate and timely evaluations, treatment documentation, and progress notes using Epic EMR.
  • Participate in interdisciplinary discussions, treatment planning, and discharge coordination when appropriate.
  • Follow established clinical protocols, patient safety standards, and applicable regulatory requirements.
  • Participate in weekend coverage approximately every 6–7 weeks and occasional holiday coverage as required.
  • Adapt to changing patient volumes and clinical priorities while maintaining efficient rehabilitation workflows.
  • Contribute to a collaborative, patient-centered rehabilitation environment focused on improving functional outcomes and community health.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
