Job Description

Northbay Integrative Primary Care believes that lasting health is created through sustainable lifestyle change, meaningful patient partnerships, and whole-person care. The Lifestyle Medicine Health Coach/Nutritionist serves as a trusted guide who empowers individuals to build healthier habits that improve quality of life, reduce chronic disease risk, and support long-term well-being. Success in this role is measured not only by knowledge shared, but by patients' ability to create lasting, meaningful changes that enhance their health and vitality.

At Northbay Health, the Lifestyle Medicine Health Coach Nutritionist is a key member of the Northbay Integrative Primary Care team, partnering with physicians and interdisciplinary providers to empower patients in achieving sustainable health through evidence-based lifestyle change. This role supports individuals in preventing, reversing, and managing chronic disease by guiding lasting behavior change using the principles of Lifestyle Medicine, nutrition science, and health coaching.

Unlike traditional medical nutrition therapy focused on acute illness, this position emphasizes whole-person wellness by addressing nutrition, physical activity, sleep, stress resilience, emotional well-being, social connection, and other lifestyle factors that influence long-term health outcomes. Through motivational interviewing, behavior change science, and personalized coaching, the Health Coach/Nutritionist helps patients identify barriers, build self-efficacy, and develop realistic, individualized action plans that promote lifelong healthy habits.

Working collaboratively within an integrative care model, this role supports patients through individual coaching, shared medical appointments, group education, culinary medicine programming, and coordination with providers including physicians, acupuncturists, behavioral health specialists, and movement professionals. The Health Coach/Nutritionist fosters an encouraging, patient-centered environment that inspires meaningful and sustainable lifestyle transformation.

PRIMARY JOB DUTIES

  1. Provides individualized lifestyle medicine coaching focused on sustainable behavior change, healthy nutrition, physical activity, sleep optimization, stress management, weight management, and chronic disease prevention.
  2. Partners with patients to establish meaningful health goals and develop personalized action plans that reflect their values, readiness for change, and unique life circumstances.
  3. Utilizes evidence-based coaching techniques including motivational interviewing, positive psychology, and behavior change science to support long-term success.
  4. Educates patients using practical, food-first nutrition principles that emphasize whole foods, culinary medicine, and sustainable eating patterns rather than restrictive diets.
  5. Identifies barriers to behavior change and collaborates with patients to develop realistic strategies that improve adherence and self-management.
  6. Documents patient encounters, progress toward goals, coaching interventions, and care plans accurately and timely within the electronic medical record.
  7. Collaborates closely with physicians and the interdisciplinary care team to coordinate individualized patient care plans and reinforce lifestyle interventions.
  8. Facilitates individual visits, group medical visits, workshops, and educational classes supporting lifestyle medicine principles.
  9. Assists in developing educational materials, wellness resources, and patient engagement tools that reflect current evidence and Northbay Health branding standards.
  10. Supports culinary medicine, wellness, and community outreach initiatives that promote healthy living and disease prevention.
  11. Participates in quality improvement initiatives, program development, and outcome measurement to continually enhance patient experience and clinical effectiveness.
  12. Maintains current knowledge of emerging research in lifestyle medicine, nutrition science, health coaching, and chronic disease prevention.
  13. Demonstrates exceptional communication skills while building trusting, compassionate relationships with patients, families, providers, and colleagues.
  14. Serves as an advocate for whole-person, patient-centered care that empowers individuals to become active participants in their health journey.
  15. Performs other duties as assigned.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
