About the role

We’re hiring a Care Navigator / Patient Advocate to work closely with patients and help them navigate their healthcare journey. This is a patient-facing role for someone who is deeply empathetic, highly organized, and comfortable using technology throughout the day.

This role is a strong fit for someone who loves helping people, communicates warmly and clearly, and can stay on top of tasks, follow-ups, and changing priorities in a fast-moving environment.

What you’ll do

  • Build trusting relationships with patients through consistent, supportive outreach
  • Help patients navigate appointments, referrals, care plans, and next steps
  • Provide high-touch support that makes patients feel seen, heard, and cared for
  • Coordinate across systems and workflows to keep patient needs moving forward
  • Document interactions carefully and maintain strong follow-through
  • Collaborate with a growing team to improve processes and patient experience
  • Adapt as tools, workflows, and internal systems evolve

  • Strong organizational skills and attention to detail
  • Comfort working in multiple browser tabs, platforms, and digital tools
  • Resilience and adaptability in a startup-style environment
  • Ability to balance warmth with efficiency and execution
