Location: Denver, Colorado Schedule: Full-time, Monday–Friday with rotating on-call

Your Role
As an RN Case Manager at Ovation Hospice, you’ll be a vital part of a compassionate interdisciplinary team. You will:
- Manage a caseload of hospice patients in homes, assisted living communities, and facilities across the Denver metro area
- Provide personalized, expert care with empathy and respect, including pain and symptom management
- Assess patients and develop individualized plans of care with the interdisciplinary team
- Build meaningful connections with patients and families, and educate them through the end-of-life journey
- Coordinate with physicians, facility staff, and the care team for seamless care
- Provide direction and leadership to hospice aides and care staff
- Complete timely, accurate documentation in accordance with Ovation policies and Colorado and Medicare standards
- Participate in the on-call rotation
Qualifications
- Unrestricted RN license valid in Colorado (or multistate compact license)
- Hospice, home health, or med-surg experience preferred
- Compassionate, caring nature with strong interpersonal and communication skills
- Comfortable with computers and electronic medical records
- Current CPR/BLS certification
- Valid driver’s license, auto insurance, and reliable transportation
