We are currently seeking a compassionate and experienced Registered Nurse - Immuniser to join our Aged Care at Home team on a permanent part-time basis, servicing clients across Currimundi and surrounding Sunshine Coast suburbs. This is an opportunity to make a meaningful difference while working autonomously in the community, delivering evidence-based nursing care and supporting clients to live safely and comfortably in the place they call home. About the Role As a Registered Nurse, you will provide holistic, client-centred nursing care to older people living in the community. Working collaboratively with clients, families, carers and multidisciplinary teams, you will assess, coordinate and deliver clinical services that promote wellbeing, independence and quality of life. Key responsibilities include:
Conducting comprehensive nursing assessments and developing individualised care plans
Coordinating and managing client care in partnership with clients, families and healthcare professionals
Delivering evidence-based clinical care, including wound management, medication administration and chronic disease management
Monitoring, evaluating and documenting client outcomes in accordance with professional and legislative standards
Identifying changes in client conditions and implementing appropriate interventions or referrals
Supporting preventative health, health promotion and early intervention strategies
Maintaining accurate and timely clinical documentation
Contributing to quality improvement initiatives and clinical best practice
About You You are a caring and capable Registered Nurse who enjoys working independently while remaining connected to a supportive team. You have a genuine commitment to delivering high-quality care and improving outcomes for older people in the community. To be successful, you will have:
Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
Demonstrated competence in contemporary nursing practice
Experience in community nursing, aged care, hospital or primary healthcare settings (highly regarded)
Strong assessment, clinical reasoning and care planning skills
Excellent communication, interpersonal and stakeholder engagement skills
The ability to work autonomously and collaboratively within a multidisciplinary team
Sound computer and mobile technology skills
A current Queensland Driver Licence and willingness to travel between client homes
Desirable Skills & Experience
Experience in care coordination and case management
Qualifications or experience in immunisation services and vaccine administration
Wound management and chronic disease management experience
Knowledge of the Aged Care Quality Standards and community care sector

Mandatory Requirements Applicants must have:
To be successful, applicants must have availability and willingness to work a recurring roster that involves weekday (Thursday's & Friday's) availability with the potential for additional shifts based on demand.
Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) and Pertussis (Whooping Cough)
Current National Police Check (with validity in the past 3 years)
Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
Current Queensland driver’s licence
Current CPR Certificate
Good computer and mobile device skills
