Physical Therapist Responsibilities

As a Physical Therapist, you will:

  • Evaluate residents in a skilled nursing and post-acute rehabilitation environment
  • Develop and implement individualized treatment plans
  • Perform comprehensive assessments and establish therapeutic goals
  • Deliver evidence-based therapy interventions
  • Complete evaluations, progress notes, recertifications, and discharge documentation
  • Collaborate with nursing, therapy, physicians, and interdisciplinary team members
  • Educate patients and families regarding treatment plans, mobility strategies, and home exercise programs
  • Support residents in achieving maximum functional independence and quality of life

Qualifications

Required:

  • Active California Physical Therapist (PT) License (or eligible to obtain)
  • Strong communication and documentation skills
  • Passion for patient-centered care

Preferred:

  • Skilled nursing facility (SNF), rehabilitation, or post-acute experience
  • Experience treating geriatric and medically complex populations
