Wij zijn op zoek naar een Casemanager die durft te doen wat nodig is. Ben jij iemand die creatieve oplossingen ziet in situaties waar anderen vastlopen? Werk je graag outreachend en vind je het belangrijk om het verschil te maken voor mensen die ondersteuning soms afwijzen maar wél nodig hebben? Dan is deze rol binnen Mozaïk Rotterdam echt iets voor jou.

Wat ga je doen?

Als Casemanager begeleid je volwassenen met meerdere uitdagingen tegelijk: (licht) verstandelijke beperkingen, psychiatrische problemen, forensische achtergrond, verslaving of dak- en thuisloosheid. Je beweegt je in het leefgebied van de cliënt en weet contact te maken ook als dat moeilijk gaat.

Jouw focus ligt op herstel en stabilisatie in het dagelijks leven. Denk aan:

  • Samen met cliënten doelen formuleren en vervolgstappen monitoren
  • Het sociale netwerk vergroten en behouden
  • Coördineren van zorg en samenwerking met behandelaren en ketenpartners
  • Administratie en rapportage zorgvuldig bijhouden in het dossier

Je werkt zelfstandig én maakt deel uit van een betrokken team. Je ondersteunt cliënten thuis, op straat of in het sociaal domein altijd met respect voor hun tempo en eigen keuzes.

  • Brutosalaris van € 3.560,- tot € 4.716,- in schaal 50 cao GG) op basis van 36 uur per week

  • Reiskostenvergoeding van € 0,38 per kilometer bij ambulant werken

Wie ben jij?

  • Je hebt een hbo-diploma zoals Social Work, SPH, Pedagogiek of vergelijkbaar
  • Je hebt ervaring in complexe zorgsettings (GGZ, LVB, forensisch, verslaving)
  • Je begrijpt psychiatrische problematiek en/of LVB-gerelateerde zorgvragen
  • Je werkt oplossingsgericht, schakelt snel en kent het sociaal domein
  • Je bent zelfstandig, toegankelijk én stevig in je communicatie
  • Je hebt een rijbewijs en bij voorkeur eigen vervoer

Ervaringsdeskundigen met een relevant diploma en ervaring met bemoeizorg, justitie, dakloosheid of verslaving worden ook van harte uitgenodigd om te reageren.
