Practice Locations The Woodlands Office129 Vision Park Blvd, Suite 109The Woodlands, TX 77384 Conroe Office500 Medical Center Blvd, Suite 280Conroe, TX 77304 What You'll Do As a Nephrology Nurse Practitioner, you will play a key role in the management of patients with chronic kidney disease, hypertension, dialysis needs, and other nephrology-related conditions. Responsibilities include:
Conduct patient assessments and develop treatment plans Round daily at dialysis facilities Participate in hospital rounding and inpatient care Collaborate closely with nephrologists and interdisciplinary care teams Educate patients and families on treatment plans and disease management Maintain accurate and timely medical documentation Deliver high-quality, compassionate patient care across multiple care settings
Schedule
Full-time position Monday through Friday Mainly clinic and dialysis coverage
1:4 weekends of hospital rounding anticipated
Local travel required between practice sites and care locations
Qualifications
Licensed Nurse Practitioner (NP) Current or eligible Texas licensure Strong clinical assessment, diagnostic, and patient management skills Excellent communication and relationship-building abilities Ability to work independently while collaborating with a multidisciplinary team Interest in nephrology and long-term patient care
