As an Admissions RN, you will play a vital role in Keeping Our Community Home by guiding patients and families through the admission process, ensuring a seamless transition to care, and helping them access the services they need with dignity and compassion.

What You'll Do

  • Conduct admission visits for home health and hospice patients.
  • Complete comprehensive assessments to determine eligibility and care needs.
  • Educate patients, families, and caregivers on available services, the plan of care, and expectations for treatment.
  • Coordinate referrals and admissions with physicians, hospitals, facilities, and community partners.
  • Obtain and process physician orders and required documentation.
  • Establish initial plans of care and facilitate a smooth transition to services.
  • Collaborate with interdisciplinary team members to ensure timely and appropriate patient care.
  • Maintain accurate and timely clinical documentation in accordance with agency, state, federal, and accreditation requirements.
  • Serve as a trusted clinical resource for patients and families during the admission process.

What You Bring

  • Current, unrestricted Registered Nurse (RN) license.
  • Home Health and/or Hospice experience preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, relationship-building, and customer service skills.
  • Ability to work independently while managing multiple priorities.
  • Valid driver's license, reliable transportation, and current automobile insurance.
  • Commitment to delivering exceptional patient-centered care.
