Clinical Services Manager (RN)

Location: Preceptor North Position Type: Full Time Wage: $80,000-$90,000/year

**10,000.00 sign on bonus**

Are you an experienced and compassionate Registered Nurse (RN) ready to lead and support a high-performing clinical team? Join Preceptor Home Health & Hospice, a trusted provider dedicated to delivering high-quality, patient-centered care. As a Clinical Services Manager, you will oversee care coordination, guide clinical staff, and ensure exceptional service delivery across home health and hospice programs.

What You’ll Do

As a Clinical Services Manager, you will provide leadership and oversight to ensure quality care and efficient operations. Responsibilities include:

  • Managing patient referrals, reviewing cases, and assigning appropriate clinical staff
  • Overseeing the development, implementation, and updates of individualized plans of care
  • Collaborating with physicians and the Medical Director regarding patient eligibility and care needs
  • Supporting clinicians with goal setting, care planning, and clinical decision-making
  • Triaging and addressing concerns from patients, families, facilities, and providers
  • Approving and managing clinical workflows for home health and hospice services
  • Conducting home visits, record reviews, and performance evaluations to ensure quality care
  • Leading and participating in interdisciplinary team (IDG) meetings and case conferences
  • Performing chart audits, supervisory visits, and quality improvement initiatives (QAPI)
  • Supporting hiring, onboarding, and development of clinical staff
  • Reviewing DME and pharmacy orders for accuracy and appropriateness
  • Monitoring outcomes and educating staff to support performance improvement
  • Managing compliance processes such as Face-to-Face (F2F) visits
  • Participating in leadership on-call rotation and completing visits as needed

What We’re Looking For

  • Current Registered Nurse (RN) license in the State of Wisconsin
  • BSN preferred
  • Home health or hospice experience preferred
  • Minimum of 2 years of leadership or supervisory experience preferred
  • Strong clinical judgment, communication, and organizational skills
  • Ability to lead and collaborate within an interdisciplinary team
  • Understanding of hospice philosophy and end-of-life care
  • Knowledge of pain and symptom management
  • Ability to manage multiple priorities in a fast-paced environment
  • Valid driver’s license with a reliable, insured vehicle
