Position Overview
RN Case Manager Signature Healthcare at Home, part of The Pennant Group, is seeking a full‑time, non‑exempt Registered Nurse Case Manager to support our west‑side Portland service area. As an RN Case Manager, you will manage a balanced caseload, provide skilled nursing interventions, coordinate interdisciplinary services, and guide patients and families through recovery, chronic‑care management, and safe transitions.

Key Responsibilities
RN Responsibilities
- Complete comprehensive nursing assessments and develop individualized care plans.
- Provide skilled nursing interventions in patient homes, including wound care, medication management, chronic‑condition support, and patient education.
- Manage a field‑based caseload with strong organizational efficiency.
- Coordinate care with interdisciplinary team members to ensure seamless service delivery.
- Communicate effectively with physicians, caregivers, and internal team members.
- Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
- Support safe transitions of care from hospitals, SNFs, and outpatient settings.
Qualifications
RN Requirements
- Current Oregon RN license (or ability to obtain).
- One year of nursing experience preferred; home‑health experience a plus.
- Strong assessment, communication, and critical‑thinking skills.
- Ability to work independently in a field‑based role.
- Reliable transportation and valid driver’s license.

West Side Portland This position primarily serves the west‑side Portland region, including neighborhoods such as Beaverton, Tigard, Hillsboro, and surrounding communities. Clinicians enjoy a diverse patient population, manageable travel zones, and strong team support.
