Werk je graag in een huiselijke, groene omgeving waar je écht de tijd hebt voor ouder wordende bewoners met een dementieel beeld? Op woongroep De Deel (Groot Schutterhoef) begeleid je maximaal tien bewoners die ieder in een andere fase zitten. Jij leest kleine signalen, biedt veiligheid en maakt van kleine momenten iets bijzonders.

Locatie: De Deel & De Hofstee op Groot Schutterhoef Groot Schutterhoef is een kleinschalige woonvoorziening met vier woongroepen van elk tien bewoners, midden in een rustige, groene buurt tussen Leusden en Amersfoort. De setting is huiselijk, overzichtelijk en gericht op 24-uursbegeleiding en het vasthouden of ontwikkelen van vaardigheden. De locatie is goed bereikbaar met het openbaar vervoer. Dagbesteding sluit aan op de woonlocatie, waardoor je met collega’s korte lijnen houdt rond ritme, structuur en activering.

Jouw werk Op De Deel en De Hofstee wonen maximaal tien ouder wordende bewoners waar o.a. het dementieel beeld en de toenemende zorgvragen onderdeel van het dagelijks leven zijn. Zelfredzaamheid staat voorop: jij stimuleert wat nog kan, en neemt stap voor stap over als het nodig is. Je werkt nauw samen met verwanten en een multidisciplinair team, zodat wonen op De Deel en De Hofstee zo aangenaam mogelijk blijft.

Wat je doet • Bieden van intensieve, persoonsgerichte (ADL-)zorg en – waar bevoegd – medicatiezorg; • Lezen van non-verbale signalen en daarop rustig, de-escalerend handelen; • Structuur, dagritme en belevingsgerichte activiteiten organiseren (binnen en in/om de boerderij); • Samenwerken met collega’s, verwanten en betrokken disciplines voor continuïteit en kwaliteit; • Observaties en voortgang methodisch vastleggen in het ECD en teamoverleggen voorbereiden.

Wie jij bent • Je hebt een mbo‑diploma Zorg & Welzijn niveau 3 of 4 (bijv. MMZ/Verzorgende IG); • Je hebt affiniteit met ouder wordende bewoners met dementie binnen EMB/VB, ervaring is een pré; • Je werkt belevingsgericht, ziet kleine stapjes en borgt rust en voorspelbaarheid; • Je kunt schakelen tussen stimuleren en overnemen, altijd met aandacht voor eigen regie; • Je bent flexibel inzetbaar in onregelmatige diensten (dag/avond/weekend/slaap).

Wat krijg je van ons • Salaris volgens FWG 35: € 2.649 – € 3.708 bruto p/m (36 uur), plus onregelmatigheidstoeslag; • Direct een contract voor onbepaalde tijd; tenzij er redenen zijn om hiervan af te wijken. Het aantal uur per week is in overleg; • Scholing en coaching gericht op ouderenzorg binnen de GHZ en belevingsgerichte methodiek; • Vitaliteitsaanbod en praktische ondersteuning zodat jij goed in je vel zit.
