Jouw rol

Je werkt ambulant binnen de langdurige zorg (Wlz) én het sociaal domein (Wmo). We werken vanuit een integrale aanpak waarbij we niet naar beperkingen kijken, maar naar mogelijkheden. Daarbij zetten we ons dienstverleningsconcept Mijn Netwerk in, waarin maatwerk, eigen regie en zelfredzaamheid vooropstaan.

Dit betekent dat je:

  • Individuele begeleiding biedt, maar ook gebruikt maakt van digitale ondersteuning via DigiContact en ondersteunt bij groepsmomenten, zoals op onze ontmoetingsplekken en tijdens cursussen.
  • Samenwerkt met gemeenten, ketenpartners en de sociale basis om ondersteuning optimaal te organiseren.
  • Durft te ondersteunen én los te laten, met oog voor wat écht nodig is (en niet alleen waar iemand ‘recht op heeft’).
  • Om kunt gaan met tegenstrijdige belangen binnen een dynamisch werkveld.

Jouw werkplek

Je wordt onderdeel van het ambulante team en bent werkzaam in Parkstad, Sittard-Geleen en/of Maastricht-Heuvelland. Je werkplek is veelzijdig: soms werk je met je kernteam Ambulant van Philadelphia, dan weer met collega’s van zorg wonen en regelmatig in groter verband binnen het sociaal domein, samen met collega’s van andere zorgaanbieders. Daarbij werk je dicht bij de leefwereld van de cliënt. Je ondersteunt cliënten met een licht verstandelijke beperking (LVB), vaak in combinatie met bijvoorbeeld autisme, verslaving of psychiatrische problematiek.

Je komt terecht in een betrokken en divers team met collega’s die extra ervaring meebrengen vanuit onder andere de GGZ en forensische zorg. Binnen het team krijg je volop ruimte om je te ontwikkelen. Zo bieden we alle collega’s scholing aan in methodisch begeleiden, met specifieke aandacht voor het werken met mensen met autisme (ASS) en het herkennen van psychiatrische ziektebeelden in combinatie met LVB. Op die manier zorgen we samen voor passende en professionele ondersteuning.

Over jou

Mensen omschrijven jou als een positief en open persoon die moeiteloos gelijkwaardigheid uitstraalt. Je weet humor in te zetten (want soms is relativeren dé sleutel) en je bent flexibel in denken en doen:

  • HBO werk- en denkniveau (Zorg & Welzijn).
  • Ervaring binnen de gehandicaptenzorg en/of ggz is mooi meegenomen.
  • Kennis van de sociale kaart van Zuid-Limburg is een pre.
  • Je hebt sterke schouders en schoenen waar je stevig in staat.
  • Je laat je niet snel uit het veld slaan: rust en overzicht houden is jouw tweede natuur.
  • Je vindt het niet erg om af en toe ’s avonds of in het weekend te werken.
  • Je beschikt over een rijbewijs en auto (je werkgebied is immers Limburgs uitgestrekt).
