Jouw locatie

Op De Mar in Sneek wonen zo’n 50 mensen verdeeld over zes woningen met ieder acht of negen appartementen. De bewoners hebben een ernstige verstandelijke of meervoudige beperking en/of moeilijk verstaanbaar gedrag. Iedereen heeft een eigen woon-, slaap- en badkamer en er zijn gezamenlijke woonkamers, een grote tuin en een snoezelruimte. Ook dagbesteding vindt op de locatie plaats.

Jouw team

Als coördinerend begeleider werk je op een woning waar begeleiding en verzorging hand in hand gaan. Je werkt samen met collega’s, andere betrokkenen en disciplines rondom de bewoners. Samen zoek je steeds naar wat iemand nodig heeft en hoe je daar als team zo goed mogelijk op aansluit.

Jouw uitdaging

Op De Mar is ongeveer de helft van je werk gericht op begeleiding en de andere helft op verzorging. Je ondersteunt bewoners bij hun dagelijkse leven, biedt structuur en nabijheid en sluit aan bij wat iemand op dat moment nodig heeft. ADL en ondersteuning bij voeding kunnen onderdeel zijn van je werk. Geen ingewikkelde woorden nodig: soms is het vooral goed kijken, aanvoelen en weten wanneer je juist wél of níét iets moet doen.

Als coördinerend begeleider houd je daarnaast het overzicht rondom jouw bewoners. Je werkt aan ondersteuningsplannen en doelen, bewaakt de voortgang en kwaliteit en stemt af met het netwerk en andere betrokken disciplines. Ook heb je een coachende rol richting collega’s en bespreek je samen wat goed gaat en waar kansen liggen.

We werken vanuit Triple-C en kijken daarbij naar de mogelijkheden van de bewoner en naar de behoefte achter het gedrag. Juist daarin komen liefde, lef en meesterschap samen. Het doel? Dat kleine momenten groot kunnen worden: meer contact, meer vertrouwen of gewoon een fijne dag. Dat is waar je het uiteindelijk voor doet.

Wie ben jij?

Je bent iemand die rustig blijft wanneer situaties veranderen, goed kijkt naar wat er écht gebeurt en collega's daarin meeneemt. Je bent communicatief sterk, houdt overzicht en durft verantwoordelijkheid te nemen voor de kwaliteit van zorg.

  • Je beschikt over goede communicatieve en administratieve vaardigheden.
  • Je kunt goed werken met digitale systemen, zoals Outlook en het ECD.
  • Je bent flexibel en veerkrachtig in een doelgroep met complexe zorgvragen.
  • Je hebt ervaring met of affiniteit met moeilijk verstaanbaar gedrag en/of intensieve verzorging.

Je bent bereid om onregelmatige diensten te werken, zoals dag-, avond-, weekend- en slaapdiensten.

Wat krijg je van ons?

  • Een brutosalaris van € 3.052 tot € 4.262 per maand (FWG 45, op basis van 36 uur).
  • Een vast contract of uitzicht daarop.
