Locatie

Op locatie De Verbinding in Arnhem Zuid begeleid je cliënten die zelfstandig wonen aan de Eimerssingel en in Schuytgraaf. Zij hebben een licht verstandelijke beperking (LVB) en/of psychiatrische kwetsbaarheid en verschillen in zelfstandigheid en ondersteuningsvraag.

Je biedt begeleiding bij cliënten thuis of op onze ontmoetingsplek. Soms staat een afspraak al gepland, maar het komt ook voor dat iemand onverwacht ondersteuning nodig heeft. Je sluit aan bij wat er op dat moment speelt en helpt cliënten om zo zelfstandig mogelijk te leven en deel te nemen aan de maatschappij.

Jouw team

Als persoonlijk begeleider LVB werk je in een divers, open en gezellig team waarin eerlijkheid en vertrouwen centraal staan. Je werkt nauw samen met het ambulante team en andere betrokken zorgpartners. Samen zorgen jullie ervoor dat cliënten de ondersteuning krijgen die bij hen past.

Je hebt veel vrijheid om je eigen werkzaamheden en afspraken in te delen. Dat vraagt om zelfstandigheid, maar biedt ook ruimte om maatwerk te leveren.

Jouw uitdaging

Als persoonlijk begeleider LVB combineer je dagelijkse begeleiding met een coördinerende rol. Je ondersteunt cliënten bij hun persoonlijke doelen, sociale contacten en het opbouwen van een netwerk. Daarbij werk je volgens de Presentiemethode: je sluit aan bij de leefwereld van de cliënt en kijkt samen wat nodig is.

Voor de cliënten waarvoor jij verantwoordelijk bent, stel je ondersteuningsplannen op en bewaak je de voortgang. Je stemt af met collega's en andere betrokken zorgpartners en zorgt ervoor dat afspraken aansluiten bij de ondersteuningsbehoefte. Ook in complexe of onverwachte situaties weet je overzicht te houden en passende keuzes te maken.

Je zoekt steeds de balans tussen ondersteunen en loslaten. Juist daarin komen liefde, lef en meesterschap samen. Je bouwt aan vertrouwen, durft verantwoordelijkheid te nemen en zet jouw vakkennis in om cliënten verder te helpen. Zo stimuleer je hen om zoveel mogelijk eigen keuzes te maken en zelfstandig te blijven functioneren.

Je werkt doordeweeks van 13.00 tot 21.00 uur en in het weekend van 10.00 tot 17.00 uur. Nacht- en slaapdiensten zijn er niet. Buiten jouw werktijden kunnen cliënten bij vragen of noodsituaties gebruikmaken van DigiContact.

Wat krijg je van ons?

  • Een contract voor onbepaalde tijd met een salaris volgens FWG 45 (€ 3.113 tot € 4.347 o.b.v. 36 uur).
  • ORT, pensioen, 8% vakantiegeld en 8,33% eindejaarsuitkering. Vakantiegeld en eindejaarsuitkering kun je ook maandelijks laten uitbetalen.
  • Een repeterend rooster, zodat je ruim van tevoren weet waar je aan toe bent.
  • Voor je werk krijg je een laptop en telefoon. Ook kun je gebruikmaken van coaching, fitness en mindfulness voor je fysieke en mentale gezondheid.
  • Ruimte om je te ontwikkelen met trainingen en coaching, onder andere gericht op LVB, psychiatrische kwetsbaarheid en methodisch begeleiden.

Wie ben jij?

Je staat stevig in je schoenen, bent betrokken en weet goed aan te sluiten bij verschillende ondersteuningsvragen. Je werkt zelfstandig, schakelt makkelijk bij onverwachte situaties en kunt ook in complexe situaties passende begeleiding bieden. Daarbij heb je oog voor samenwerking, kwaliteit en de eigen regie van cliënten.

Wat je verder meebrengt:

  • Minimaal een mbo-4 diploma in Zorg & Welzijn en hbo werk- en denkniveau.
  • Ervaring met LVB, GGZ-problematiek en maatschappelijke ondersteuningsvragen.
  • Goede communicatieve en administratieve vaardigheden en het vermogen om methodisch te werken.
  • Een ondernemende houding, empathie en een gezonde dosis humor.
