Willkommen

in unserem modernen und großzügigen Hausarztzentrum Am Kreuz in Bayreuth. Gemeinsam mit unseren Kolleginnen aus der Kinderheilkunde und Jugendmedizin versorgen wir am Standort im Sinne der Familienmedizin einen gewachsenen Patientenstamm. Zur Erweiterung und Verstärkung unseres Teams suchen wir zum nächstmöglichen Zeitpunkt einen engagierten Diabetesberater DDG (m/w/d). In unserer modernen Praxis erwartet Sie eine abwechslungsreiche und verantwortungsvolle Tätigkeit in einem freundlichen, kollegialen Umfeld.

Ihre Aufgaben

  • Durchführung von Schulungen für Patienten mit Diabetes Typ 1 und Typ 2 – auch telemedizinisch
  • Individuelle diabetologische Beratung sowie Ernährungsberatung
  • Betreuung von Typ-1-Diabetes-Patienten mit CSII-Anwendung (Insulinpumpentherapie)
  • Mitbetreuung von Patienten im Rahmen des DMP-Programms
  • Beratung von Neupatienten, insbesondere hinsichtlich CSII und CGM (kontinuierliche Glukosemessung)

Ihr Profil

  • Abgeschlossene Weiterbildung zum/zur Diabetesberater/in (DDG)
  • Hohes Maß an Einfühlungsvermögen, Empathie und Geduld im Umgang mit Patienten
  • Aufgeschlossene und freundliche Persönlichkeit
  • Selbstständige, strukturierte und verantwortungsvolle Arbeitsweise
  • Zuverlässigkeit und Teamfähigkeit
  • Interesse an Weiterentwicklung und Fortbildungen im diabetologischen Bereich
  • Bereitschaft zur Mitgestaltung moderner, zukunftsorientierter Patientenversorgung, z. B. durch den Einsatz von Videosprechstunden und digitalen Lösungen der Patientenversorgung
