With a staff of 950+ and an annual budget of $140 million, Project Renewal is one of the larger social service nonprofits in New York City.   Title: Team Leader Program: Canopy House Women’s Shelter Salary: $57,000 - $62,000   Program Description: Canopy House provides 171 emergency shelter beds for homeless women diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities, occupational therapy, mental health services and housing placement assistance. Canopy House is a unique facility in that it is co-located with a supportive housing program that occupies the top 15 floors of the building and offers a street facing Article 28 clinic on the first floor dedicated to providing primary and behavioral health care services.   Essential Duties and Responsibilities: The essential duties of the Team Leader include but are not limited to the following activities:

In addition to supervising Case Managers, maintains a caseload of 10 to 15 clients. Follows up on referrals to determine if they meet the criteria for admission to the shelter and verify their appropriateness for referral to various programs.  Assists the Director of Social Services in providing guidance and direction to the Case Managers to ensure assessments--including psycho-social evaluations--are being performed, Independent Living Plans are being promptly followed, referrals to appropriate programs are being made, and integration of services is occurring. Assists the Director of Social Services and Clinical Coordinator in conducting chart audits to ensure all required documents have been given to and acknowledged by clients, and in coordinating the delivery of services to clients. Provides individual and group sessions to assist the clients in achieving independence, obtaining permanent housing and returning to their communities. May, as a part of aftercare service, provide outreach to the clients and housing providers to ensure clients are performing well in their new living accommodations and being successful in their reintegration into the community, and may also provide assistance as may be needed to ensure the clients’ success. Ensures that accurate and up to date clinical documents are being maintained and that required data is input into the CARES system by Case Managers in a timely fashion. Attends and participates in weekly Case Conference meetings on the status of each client. 

Performs other duties as assigned by the Director of Social Services.

  Physical Activities: While performing the duties of this position, the Program Director must stand to talk to clients and staff, sit for extended periods of time at the computer, and reach or squat to get records from the files.    Experience Requirements

Bachelor’s degree from an accredited college or university is preferred with at least one year of experience working with homeless, mentally ill and/or chemically addicted individuals or other special needs population. Associate’s degree with two years of experience working with homeless, mentally ill and/or chemically addicted individuals or other special needs populations. A High School Diploma or GED equivalent with three years of experience working with homeless, mentally ill and/or chemically addicted individuals or other special needs populations. Bilingual or multilingual a plus Must have proven supervisory skills

  Preferred skills:

Team Building:  An ability to motivate all staff and to work collaboratively with staff from other programs to provide the most beneficial services to the clients. Communication: Excellent oral, writing and listening skills. Organizational skills: An ability to work independently and to initiate actions to support the efforts of the staff and the clients. Interpersonal skills: An ability to relate with clients as well as all levels of staff. Computer skills: Knowledge of case management software’s as well as a proficiency in Microsoft Office Suite. Language: Bilingual or multilingual a plus
