Administer medications to individuals and follow the five rights for verification purposes Follow physician orders and monitor residents for changes in health, behavior, injuries, infections, seizures, or other medical concerns; report and documents as required. Attend medical and dental appointments and communicate with physicians, Regional Center, day programs, and other service providers. Maintain accurate medical records, medication logs, appointment calendars, incident reports, and daily client notes. Provide supervision, care, and training to residents in accordance with their IPP, Behavior Support Plans, and facility procedures. Oversee daily household operations, including meals, shopping, cleanliness, supplies, appointments, and completion of assigned tasks. Support and guide DSP staff to ensure quality care, safety, professionalism, and resident independence. Assist with employee coaching, corrective action plans, training, and performance guidance as needed. Provide transportation for medical appointments, community outings, and other activities as outlined in the IPP. Participate in community activities and outings with residents. Respond appropriately to behavioral or medical emergencies and implement emergency procedures when needed. Maintain a safe, organized, and supportive home environment while ensuring compliance with facility standards. Work an alternating schedule with the Administrator, including weekends. Perform other related duties and assignments as required.
Education and Experience:
High school diploma or GED. Must possess a valid California Licensed Vocational Nurse (LVN) license. DSP I and DSP II certificates. Current CPR/First Aid certification. CPI (Crisis Prevention Intervention) certification preferred or willingness to obtain. At least six months of experience working as an LVN in a residential care facility or similar setting preferred. At least one year of experience supporting adults with intellectual and developmental disabilities preferred. Experience with medication administration, G-tube care, injections, wound care, and other nursing procedures preferred.
Physical Requirements:
The LVN / House Lead position will typically work in a residential, office, and community-based environment, with occasional work outdoors and in small or enclosed spaces. Frequent sitting, standing, walking, grasping, feeling, talking, hearing, and repetitive motions. Occasional lifting of up to 50 lbs. Ability to bend, twist, stoop, squat, push, pull, reach, kneel, crouch, and crawl as needed. Ability to assist individuals with activities of daily living and personal care. Ability to travel between work locations and community activities as required.
