Als transferverpleegkundige/zorgbemiddelaar ben jij de cruciale schakel in het overplaatsingsproces van onze patiënten. Plannen, coördineren en organiseren van complexe zorgtrajecten? Daar krijg je energie van! Benieuwd hoe een dag eruit kan zien? Lees dan snel verder.

In de ochtend start je met je collega’s en bespreek je de workload en de nieuwe aanvragen. Eén van de nieuwe aanvragen betreft de nazorg van een patiënt naar een revalidatievoorziening. Aan jou de taak om de nazorg te regelen. Je gaat bij de patiënt langs op de verpleegafdeling en bespreekt zijn of haar situatie, wensen en mogelijkheden. Samen met de informatie van de verpleegkundigen en de behandelaar, breng je de benodigde nazorg in kaart. Je maakt als het ware de puzzel: wat heeft de patiënt nodig en welke vervolgzorg sluit daar het beste bij aan? Je stemt hierover af met de patiënt en familie. Vervolgens onderzoek je welke passende vervolgzorg beschikbaar is en zet je de aanvraag uit bij zorgaanbieders in de regio. Daarbij krijg je te maken met een dynamisch speelveld van beschikbare plekken, passende zorg en de wensen van de patiënt. Het is aan jou om hierin steeds de juiste balans te vinden.

Je houdt je bezig met het organiseren en coördineren van nazorgtrajecten, zoals (geriatrische) revalidatiezorg, thuiszorg, VVT en samenwerking met verschillende ketenpartners. Jij hebt de regie over het hele transferproces. Je coördineert de verschillende stappen, bewaakt de voortgang en zorgt ervoor dat alle betrokken partijen op tijd over de juiste informatie beschikken. Zo zorg jij ervoor dat de patiënt zo soepel en passend mogelijk kan doorstromen naar de volgende fase van de zorg. Jij laat de patiënt het ziekenhuis uitgaan met kwalitatief goede nazorg. Dit doe je niet alleen, maar samen met het team Bureau Transmurale Zorg.

Wij bieden een tijdelijk contract van 12 maanden voor 32 uur per week. Bij een goede samenwerking kijken we graag samen naar de mogelijkheden voor de toekomst.

Wij vragen

  • Je bent gediplomeerd verpleegkundige en hebt hbo-werk- en denkniveau;
  • Een afgeronde hbo-V-opleiding is een pré.
