Stichting Amsterdamse Gezondheidscentra (SAG) zoekt een:

POH-S (Praktijkondersteuner Somatiek)

  • Voor 16 tot 24 uur per week in Gezondheidscentrum Oosthoek

SAG streeft naar multidisciplinaire samenwerking met andere zorgprofessionals. Heb jij een proactieve houding en pak je graag de regierol? Kijk je niet alleen naar de medische kant, maar juist ook naar het sociaal welzijn van de patiënt? Zoek je actief de samenwerking op? Dan zoeken wij jou!

De groeiende groep oudere patiënten die zo lang mogelijk zelfstandig thuis blijft wonen, krijgt vaak te maken met meerdere zorgprofessionals. Om volwaardige chronische zorg te kunnen leveren, heb jij als POH-S een centrale rol in onze praktijk. Je bent het eerste aanspreekpunt voor de chronische patiënten binnen de ketenzorg-programma’s Astma, COPD, CVRM en Diabetes.

Jouw profiel:

  • Je bent in het bezit van het diploma POH-Somatiek of je hebt een achtergrond als HBO-verpleegkundige / HBO- zorgverlener en bent bereid de opleiding (eventueel verkort) te volgen;
  • Je kunt zelfstandig de chronische zorg uitvoeren;
  • Je omarmt de dynamiek van samenwerken met collega’s in de gezondheidscentra en met de diverse externe partners in de wijk;
  • Je kunt goed motiveren, organiseren en coördineren en bent patiëntgericht ingesteld;
  • Het delen van kennis en ervaring binnen het vakinhoudelijk overleg en intervisiegroepen is voor jou geen ‘moetje’ maar een vanzelfsprekendheid.

Wij bieden:

  • Bruto maandsalaris volgens de CAO Huisartsenzorg, tussen €3.795,- en €4.977,- bij een fulltime dienstverband van 38 uur per week;
  • Meerkeuze Arbeidsvoorwaarden, pensioen, vakantiegeld, eindejaarsuitkering van 8,33%;
  • De VOG die we van je verlangen als je bij ons in dienst treedt, wordt door SAG vergoed.
