Das Sanatorium Hera ist ein renommiertes Privatkrankenhaus der KFA Wien und bietet Patientinnen und Patienten erstklassige medizinische Betreuung, spezialisierte Fachambulanzen und persönliche Versorgung in zentraler Wiener Lage - mit einem engagiertem Team und einem Fokus auf Qualität und Menschlichkeit. 1 Entlassungsmanager*in ab sofort, 40h/Woche

Aufgabenbereich

* Eigenständige Planung, Organisation und Koordination der poststationären Versorgung im Rahmen der Entlassungsmanagements von Patient*innen in enger Zusammenarbeit mit den behandelnden Ärzt*innen, dem Pflegepersonal

und den relevanten interprofessionellen Berufsgruppen

* Frühzeitige Einschätzung des individuellen Unterstützungs- und Versorgungsbedarfs von Patient*innen zur Sicherstellung einer bedarfsgerechten und kontinuierlichen Versorgung nach dem stationären Aufenthalt

* Beratung, Information und Unterstützung von Patient*innen und deren Angehörigen hinsichtlich geeigneter Versorgungsangebote und Unterstützungsleistungen

Qualifikationen

* Sicherstellung eines strukturierten Informationsflusses und einer bedarfsgerechten Weitergabe entlassungsrelevanter Informationen an alle beteiligten internen und externen Berufsgruppen

* Mitwirkung an der Weiterentwicklung und Optimierung der hausinternen Prozesse des Entlassungsmanagements

Qualifikationen

* Eine nach dem österreichischen Gesundheits- und Krankenpflegegesetz (GuKG) anerkannte Berufsberechtigung für den gehobenen Dienst für Gesundheits- und Krankenpflege mit einer nachweislichen Gesundheitsberufe-Registrierung

* Weiterbildung im Bereich Case & Care Management von Vorteil

* Bereitschaft zur kontinuierlichen Fort- und Weiterbildung, um stets auf dem aktuellen Stand der Wissenschaft zu bilden

* Gute Deutschkenntnisse in Wort und Schrift

* Ausgeprägtes Organisationsvermögen sowie eine strukturierte und vorausschauende Arbeitsweise

* Hohe soziale Kompetenz, Kommunikationsfähigkeit und wertschätzender

Umgang mit Patient*innen, Angehörigen und Mitarbeiter*innen

Das kollektivvertragliche Mindestentgelt inklusive Zulagen und Nebengebühren beträgt zwischen EUR 55.226,24 jährlich brutto (Basis 40 Stunden/Woche). Das tatsächliche Bruttogehalt berechnet sich auf der Basis Ihrer Ausbildung, Qualifikation und Berufserfahrung.
