Dipl. Pflegefachfrau/\-mann 50 \-100% mit oder ohne Fallführung

Deine Aufgaben

  • Bei unseren Kundinnen und Kunden übernimmst du Einsätze in der Grund\- und Behandlungspflege.
  • Du spielst eine massgebliche Rolle dabei, sicherzustellen, dass unsere Kundinnen und Kunden zu Hause eine erstklassige und individuelle Pflege geniessen.
  • Du berätst und begleitest unsere Kundinnen und Kunden und deren Angehörigen, anhand ihrer individuellen Bedürfnissen.
  • Du führst Bedarfsabklärungen mit RAI\-HC beim Kunden durch.
  • Du wirkst bei präventiven, diagnostischen und therapeutischen Massnahmen mit.
  • Förderung der Selbstständigkeit und Lebensqualität der Kundinnen und Kunden im häuslichen Umfeld
  • Du führst umfassende Pflegeanamnesen und Assessments durch.
  • Du leitest die unterstellten Mitarbeitenden in Bezug auf pflegerische Massnahmen an.
  • Du übernimmst administrative Aufgaben im Rahmen der Fallführung.

Das bringst du mit

  • Du bist diplomierte Pflegefachperson (HF, FH, AKP, DN II).
  • Du verfügst über den InterRai HC Kurs oder hast die Bereitschaft diesen zu absolvieren.
  • Du verfügst idealerweise über eine mehrjährige Berufserfahrung in der Langzeitpflege bzw. im Spitex\-Bereich
  • Selbstständiges, strukturiertes und effizientes Arbeiten zeichnen dich aus.
  • Du bist belastbar und flexibel.
  • Du verfügst über den Fahrausweis der Kategorie B und hast idealerweise ein eigenes Auto.
