Program Services Clinical Coordinator As a Clinical Coordinator (QIDP), you’ll oversee care for adults with intellectual and developmental disabilities who live with Mentors in our Host Home (shared living) program. This isn’t a desk job. You’ll spend much of your week in the community, visiting homes, guiding Mentors, and partnering with guardians and service coordinators so each person gets consistent, quality support. The Role at a Glance
Full-time, salaried, Monday–Friday, 8am–5pm Field-based, with home visits scheduled throughout the month across [Springfield and the surrounding service area] Starting salary: $45,000/year
What a Typical Week Looks Like
Conduct scheduled home visits with individuals and their Mentors (host home providers) Answer Mentors’ questions, give guidance, and provide training when needed Attend quarterly and annual review meetings with service coordinators and guardians Gather monthly data and complete a Monthly Progress Report for each individual Collect documentation from host homes, upload it, and keep records organized and complete Check in medications at the end of each month, deliver them to homes, and review medication documentation for accuracy Keep regular communication with guardians, service coordinators, and Mentors

Qualifications:
Bachelor’s degree in human services or related field. One year of work-related experience working with individuals with intellectual and developmental disabilities, brain injury, youth placed at-risk, etc. preferred. An equivalent combination of education and experience. Current driver's license, car registration, and auto insurance. Licensure and training as required by state such as CPR, First Aid, Behavioral Intervention Techniques, etc. QMRP/QIDDP as required by state. A reliable, responsible attitude and a compassionate approach. A commitment to quality in everything you do.
