Kompetenzen und die Rücksicht auf individuelle Bedürfnisse in der Alltagsgestaltung stehen im Vordergrund der

individuellen Wohnräume. Das Wohnkonzept in Horn ist für Menschen verschiedener Altersstufen mit unterschiedlichen

Deine Aufgaben

- Wertschätzende Pflege, Unterstützung und Begleitung von Menschen mit Behinderung

- Ansprechperson bei medizinischen und pflegerischen Fragen, sowie bei Arztvisiten

- Unterstützung beim Medikamentenmanagement und bei dem Pflegeplanungsprozess

- Entwicklung von Alltagsstrukturen mit Blick auf größtmögliche Selbstbestimmung

- Erarbeiten von Lösungen zusammen mit Ihren Kolleg:innen

- Vorausschauendes, präventives Arbeiten im Team

Dein Profil

- abgeschlossene Ausbildung als Dipl. Gesundheits- und Krankpfleger:in

- Eintrag im Gesundheitsberuferegister

- Freude an Mitgestaltung, Beratung und

- Weiterentwicklung
