Co Vás čeká:• Komunikace se zájemci o sociální službu a jejich blízkými.• Provádění sociálního šetření.• Poskytování základního a odborného sociálního poradenství v kontextu sociálních služeb.• Příjem klientů do sociální služby domova pro osoby se zdravotním postižením.• Příprava návrhu smlouvy o poskytování sociální služby včetně dodatků.• Vedení sociální dokumentace.• Metodické vedení individuálního plánování ve službě včetně zpracování, vyhodnocování a kontroly plnění individuálních plánů klientů.• Zabezpečení sociální agendy včetně řešení sociálně právních problémů ve službě a komunikace s opatrovníky a dalšími úřady.• Vytváření koncepce sociální práce.• Monitoring kvality poskytované péče.• Spolupráce na aktualizaci vnitřních předpisů organizace, pracovních postupů, metodik a standardů kvality.• Práce v IS Cygnus 2.Co od Vás požadujeme:• Vyšší odborné vzdělání nebo vysokoškolské vzdělání splňující kvalifikační požadavky dle zákona č. 108/2006 Sb., o sociálních službách, v platném znění, tj. v oboru sociální práce, sociální/speciální pedagogika, sociální politika apod.• Dobrá znalost zákona č. 108/2006 Sb., o sociálních službách a vyhlášky č. 505/2006 Sb., v platném znění, znalost standardů kvality sociálních služeb.• Trestní bezúhonnost a zdravotní způsobilost.• Praxe na obdobné pozici výhodou, ale není podmínkou.• Zkušenosti s prací s lidmi s mentálním a kombinovaným postižením výhodou.• Schopnost samostatné práce i spolupráce v týmu.• Dobré komunikační a organizační dovednosti.• Pečlivost, samostatnost a spolehlivost při plnění svěřených úkolů.• Ochota učit se novým věcem a dále se profesně rozvíjet.Co Vám nabízíme:• Pracovní poměr na dobu určitou 1 rok, s možností následného prodloužení při oboustranné spokojenosti.• Finanční ohodnocení odpovídající 10. platové třídě (výše platu je určena dle platových tabulek v závislosti na započitatelné praxi na základě Nařízení vlády č. 341/2017 Sb.) + případné osobní ohodnocení odpovídající kvalitě vykonávané práce.• Pestrou, smysluplnou a naplňující práci s možností profesního růstu.• Možnost účasti na odborných kurzech, školeních a stážích.Pokud Vás tato nabídka zaujala, pošlete nám svůj životopis spolu s krátkým motivačním dopisem na e-mail: <a href="mailto:[email protected]" rel="nofollow">[email protected]</a>, nejpozději do 31. 10. 2026 včetně, do předmětu uveďte Sociální pracovník – DOZP Zlín.Přijaté životopisy uchazečů budou uchovány pouze po dobu trvání výběrového řízení.
