Nous recherchons activement une infirmière ou un infirmier clinicien/autorisé pour combler un mandat ciblé à court terme au Centre de santé Isle-Dieu à Matagami, dans le Nord-du-Québec. Joignez la Direction des programmes sociaux (DPS) pour apporter votre expertise directement au cœur du milieu de vie des usagers.

  • Date de fin du mandat : 30 novembre 2026
  • Horaire : Temps complet de jour, formule 5/2 (du lundi au vendredi de 08 h 00 à 16 h 30)
  • Temps de repas : 1 heure
  • Garde : Aucune garde requise
  • Orientation : 2 jours complets d'orientation prévus en amont, soit le 11 et 12 novembre 2026 (pris en charge à 100 % par l'agence).

Responsabilités

  • Effectuer des visites à domicile pour évaluer l'état de santé fonctionnel, biopsychosocial et les besoins globaux des usagers en perte d'autonomie ou en post-hospitalisation.
  • Concevoir, appliquer et réévaluer les plans de soins et de traitements infirmiers personnalisés.
  • Prodiguer des soins cliniques diversifiés à domicile (pansements complexes, prélèvements, gestion des maladies chroniques, soins palliatifs).
  • Assurer l'évaluation rigoureuse du dossier de l'usager et documenter de manière exhaustive les interventions dans le système informatique.
  • Collaborer étroitement avec l'équipe interdisciplinaire du soutien à domicile et les proches aidants pour optimiser le maintien à domicile sécuritaire.

Exigences

  • Membre en règle de l'Ordre des infirmières et infirmiers du Québec (OIIQ).
  • Minimum de 2 ans d'expérience obligatoire en soutien à domicile (SAD).
  • Excellente maîtrise et habileté avec le logiciel I-CLSC.
  • Permis de conduire valide et véhicule automobile requis pour effectuer les tournées et déplacements auprès des usagers sur le territoire.
  • Grand sens de l'autonomie clinique, aisance professionnelle en isolement relatif et rigueur organisationnelle.
