📌 À propos du poste

Le CISSS de la Gaspésie recherche activement un(e) Travailleur(euse) social(e) pour un mandat de 3 mois basé au CLSC de Murdochville.

Dans ce milieu à échelle humaine, vous exercerez un rôle de pivot communautaire unique en répondant à l'ensemble des besoins psychosociaux d'une population d'environ 600 habitants. Ce poste demande une grande polyvalence clinique, de l'autonomie et une excellente collaboration avec les autres professionnels de la santé du secteur.

🎯 Vos principales responsabilités

  • Évaluer le fonctionnement social et les besoins psychosociaux de la clientèle locale (tous âges confondus).
  • Offrir du soutien, de l'orientation, de l'intervention de crise ou des suivis à court et moyen terme.
  • Collaborer de manière étroite avec les équipes médicales du CLSC et les partenaires du réseau de la Gaspésie.
  • Élaborer et mettre en œuvre des plans d'intervention individualisés ou familiaux adaptés à la réalité régionale.

📋 Exigences

  • Diplôme : Baccalauréat ou maîtrise en travail social et membre en règle de l’OTSTCFQ.
  • Logistique obligatoire : Permis de conduire valide sans restriction ET accès à une voiture personnelle. Il n'y a aucun transport en commun disponible dans ce secteur.
  • Compétences : Autonomie, polyvalence et adaptabilité aux réalités des petites communautés isolées.
