Position Summary

• Responsible for coordination of care for neurosurgery or neurology patient

population.

• Associate, Diploma, Bachelor Degree or Master’s in Nursing

Licenses and Certification

• Current Illinois RN licensure

Experience and Skills

• Technical Experience: 3 years

Physical Activities

• Intermittent hand manipulation

• Intermittent lifting and carrying of 20 lbs.

Role Specific Responsibilities

o Serves as a clinical expert to help neurosurgery or neurology patients

and families navigate the health care system.

o Provides initial coordination of care for patient receiving pre-appointment

treatment

o Reviews patient’s diagnosis, medical history, and physician disposition

and communicate information to patient.

o Directs patient to facilities listed on the Network Provider list when pre-appointment treatment is recommended and facilitates referrals to

these facilities

o Assists physicians in coordination of care of neuroscience or neurology

patients with other health care professionals.

o Refers patients to other community resources, as necessary

o Provides ongoing navigation for patients receiving pre-appointment

treatment

o Communicates written treatment completion information to the Primary

Care or referring physician if pre-treatment results in symptom resolution

and patient does not require an appointment with the Southern Illinois

Healthcare Brain and Spine Institute

o Provides on-going documentation of nurse navigation activities on each

patient, including the time involved in these activities

o Assists in the development, documentation and analysis of program

performance

o Serves as a clinical resource to Intake Specialists

o Collaborates with other department members to facilitate effective triage

of new neuroscience or neurology patients

o Eliminates unnecessary delays in clinician reviews, including supervising

the collection of patients’ diagnostic studies

o Returns Southern Illinois Healthcare Brain and Spine Institute phone

messages and other communication within 24 hours

Neurology:

o Responsible for all triaging of neurology referrals sent to Brain and Spine

o Responsible for transitional care management of discharged neurology patients including: imaging, wellness checks, referrals, appointments and medication refills/order

o Responsible for any follow up indicated on Emmi report
