Schlieren ist eine dynamische, wachsende Stadt im Limmattal mit rund 20'000 Einwohnerinnen und Einwohnern. Als zuverlässige Arbeitgeberin bietet die Stadt Schlieren ein professionelles Umfeld, um sehr gute Leistungen zu erbringen. Mit einem differenzierten Angebot an Pflege\- und Betreuungsleistungen an drei verschiedenen Standorten ermöglicht die Stadt rund 80 älteren Bewohnerinnen und Bewohnern mit unterschiedlichem Pflege\- und Betreuungsbedarf ein wohnliches Umfeld mit professionellen Dienstleistungen. Rund 120 Mitarbeitende und Lernende sorgen im Alterszentrum Sandbühl sowie in zwei verschiedenen Pflegewohnungen und im Betreuten Wohnen Bachstrasse und Mühleacker für das Wohl der älteren Menschen.

Zur Unterstützung unseres Team im Betreuten Wohnen Mühleacker suchen wir per sofort oder nach Vereinbarung eine/n

Dipl. Pflegefachperson HF 80\-100 % ===================================

Ihre Hauptaufgaben

Verantwortung für Fachaufgaben:

  • Pflegeprozess steuern, Leistungsplanung, Eintrittsgespräche, RAI LTCF, Pflegedokumentation, Bezugspersonenarbeit, Umsetzung von Pflegekonzepten
  • Wundversorgung, Medikamentenmanagement, Blutentnahmen, Durchführung Bezugspflege, Katheterisieren usw.
  • Inputs geben, Ausführungskontrolle, regelmässige Überprüfung der Tagesstruktur, Ernährungsspezifisches Assessment etc.
  • Abrechnungen, Organisation von Transporten usw.
  • Begleitung von neuen Mitarbeitenden, Beteiligung an der praktischen Ausbildung von Lernenden, definierte Aufträge in Projekten bearbeiten bzw. mitbearbeiten usw.
  • Tagesverantwortung gemäss Einsatzplan inkl. disponieren der Personalressourcen usw.
  • Massnahmen für eine ganzheitlichere Versorgung der Bewohnenden umsetzen, Gesundheitsvorsorge mit unterstützen

Zusammenarbeit/Kommunikation

  • Der Umgang mit Kolleginnen und Kollegen, Bewohnern und Bewohnerinnen, Angehörigen und Lieferanten ist wertschätzend, respektvoll und authentisch
  • Information relevanter Stellen bei heiklen oder aussergewöhnlichen Situationen
  • Aktive Zusammenarbeit im Team und interdisziplinär
  • Gewährleistung des Informationsflusses

Qualitätsmanagement

  • Umsetzen der vorgegebenen Qualitätsnormen
  • Fachgerechter Umgang mit Material und Geräten

Ihr Profil

  • Diplomausbildung in Pflege (AKP, PSYKP, DNI, DNII oder HF)
  • Ausgewiesene Fachkompetenz in der Langzeitpflege
  • Sozialkompetenz, insbesondere Kommunikationsfähigkeit
  • Teamfähigkeit
  • Organisationsgeschick
  • Einsatzbereitschaft und Flexibilität
  • Veränderungsbereitschaft und Lernfreude, Bereitschaft zur laufenden Weiterbildung
  • Gute PC\-Anwenderkenntnisse
