Voor de regio Coevorden zoeken we voor de wijk Coevorden, wijk Eendrachtshof en wijk Franciscus een HBO-verpleegkundige die stevig in de schoenen staat. In deze dynamische functie beheer je jouw eigen caseload verspreid over de drie wijkteams in Coevorden.

Wat ga je doen?

Als HBO-Wijkverpleegkundige ben jij de spil in de wijkzorg van Coevorden. Je overstijgt de dagelijkse zorg en kijkt met een helikopterview naar de situatie van de zorgvragers in de drie wijken. Je stelt zelfstandig indicaties op en coördineert de zorg rondom de zorgvragers. Verder coach je de teamleden op het gebied van kwaliteit, klinisch redeneren en zelfredzaamheid van de zorgvrager. Hierin schakel je soepel tussen de teams, Coevorden, Eendrachtshof en St. Franciscus om de zorgprocessen optimaal te stroomlijnen. Daarnaast onderhoud je nauwe contacten met huisartsen, fysiotherapeuten en maatschappelijk werk in de regio.

Wat breng je mee?

  • Je bent in het bezit van het diploma HBO-Verpleegkundige en bent BIG geregistreerd.
  • Je hebt bij voorkeur ervaring in de wijkverpleging en met het zelfstandig indiceren.
  • Je bent communicatief sterk, zowel mondeling als schriftelijk.
  • Je kunt goed overstijgend denken en bezit een gezonde dosis daadkracht.
  • Je bent flexibel ingesteld en kunt snel schakelen in onvoorziene situaties.
  • Je bent bereid om indien nodig alle voorkomende dag- en avonddiensten in de zorg te werken.
  • Je wilt een bijdrage leveren aan het levensgeluk van ouderen en een voor hen zo gelukkig mogelijke leef-tijd.
  • Je levert een bijdrage aan de kwaliteit van zorg en dienstverlening.

Wat bieden wij jou?

  • Direct een contract voor onbepaalde tijd.
  • Een werkweek van 32 tot 36 uur, in overleg in te richten.
  • Inschaling volgens FWG 50 volgens de CAO VVT op basis van je opleiding en ervaring tussen €3502,- en €4822,- bruto op basis van een fulltime dienstverband.
  • Een vaste eindejaarsuitkering van 8,33%.
  • Lidmaatschap kwaliteitsregister V&VN.
  • Toegang tot de Tangenborgh academie om je verder te ontwikkelen (o.a. e-learningen van Good Habitz).
