Svane Pleje Aarhus arbejder målrettet med borgeren i centrum og lægger stor vægt på kvalitet, samarbejde og udvikling. Vi har selvorganiserede teams, hvor ansvar og initiativ deles, og hvor faglighed og nærvær er i højsædet.

Dine primære opgaver

Som teamkoordinator får du en alsidig hverdag med ansvar for at sikre en stabil og sammenhængende daglig drift. Du bliver bindeleddet mellem kontoret, de udekørende kolleger og eksterne samarbejdspartnere som terapeuter, sygeplejersker og visitationen.

Dine kerneopgaver er blandt andet:

• Deltage i driftsopgaver hver morgen fra 07.00–11.00 med fokus på borgere med særlige behov.

• Understøtte kontorets administrative funktioner fra kl. 11.00.

• Opstarte nye borgere med journaloprettelse og korrekt dokumentation.

• Sikre faglig sparring og koordinering hos medarbejderne i driften.

• Håndtere callcenterfunktion med løbende dialog om akutte behov og ændringer.

• Deltage i og facilitere temamøder mindst én gang ugentligt.

• Oplære og være mentor for nye medarbejdere, herunder koordinerer kurser og e-læring.

• Løbende opdatere og kvalitetssikre journaler, samt koordinere borgerforløb sammen med terapeuter og øvrige samarbejdspartnere.

• Planlægge og gennemføre opfølgningssamtaler efter forløbsmodellen.

• Varetage forvagts funktion hver tredje weekend med ansvar for bemanding, sygemeldinger og dag-til-dag-planlægning.

Vi søger dig, der:

• Er uddannet social- og sundhedsassistent med erfaring fra hjemmepleje.

• Har organisatorisk flair og kan arbejde systematisk samt selvstændigt.

• Har stærke kommunikationsevner og kan samarbejde på tværs af faggrupper.

• Er fleksibel og mødestabil med et stort engagement for borgernes trivsel.

• Har kendskab til omsorgssystemer som CURA, gerne på højt niveau.
