Hiring Incentive - $1,200 Job Summary The LPN provides high-quality, patient-centered care for individuals with substance use disorders in our Walk-in Medication for Addiction Treatment (WinMAT) clinics. The LPN functions within a team-based, low-threshold care model, supporting medication treatment, patient engagement, and ongoing monitoring. The role emphasizes protocol-driven care, patient education, and coordination with providers, RNs, and interdisciplinary team members to support treatment adherence and recovery. Reports to Manager of Nursing. Essential Duties & Responsibilities · Conduct nursing visits in collaboration with the WinMAT provider, RN, and interdisciplinary care team. · Support shared visits with providers, assisting in the evaluation and management of: Medication side effects; Adherence concerns; Dose stabilization or adjustment · Collect and document objective and subjective clinical data, including: Vital signs; Substance use patterns; withdrawal symptoms; psychiatric symptoms; medication adherence and side effects; safety concerns · Provide clear, individualized patient education on: Medication for addiction treatment (MAT); treatment plans and expectations; harm reduction strategies and overdose prevention · Reinforce treatment plans and support adherence using practical strategies (e.g., reminders, behavioral cues, addressing barriers). · Perform clinical tasks within scope and under provider direction: Venipuncture; Intramuscular (IM) injections (eg IM naltrexone) and subcutaneous injection (eg long acting buprenorphine) ; Urine and oral toxicology collection · Administer medications per provider order and document all interventions accurately in the medical record. · Coordinate with pharmacies regarding: Medication refills; Prior authorizations; Patient assistance programs · Participate in daily clinical workflows: Monitor Epic/OCHIN In Basket and message pools; Support timely communication across the care team · Conduct protocol-driven phone screenings, gather clinical information, and escalate concerns outside established guidelines to the RN or provider. · Distribute safer use materials, naloxone, and testing strips as indicated. · Collaborate with internal teams, external partners, and patient support systems to facilitate access to care and continuity of treatment Other Job Duties & Responsibilities · Perform other related duties as assigned. Job Qualifications · Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Ohio · Experience working with individuals with substance use disorders preferred · CPR certification and nonviolent crisis intervention training required within 60 days of hire
