Role: Senior Case Manager, C3 Skid Row Reports to: Program Manager Department: Outreach Program: 1502-Downtown C3 Location: C3 Skid Row - 627 S. San Julian Street, Los Angeles, CA 90014  Setting: 100% Onsite Schedule: Flexible Weekdays, 7:30 AM - 4:00 PM Status: Part-Time/ Non- Exempt (hourly)/ Non-Management  Benefits: Medical, Vision, Dental, Life Insurance, 403(b) Retirement plan, Employee Assistance Program (EAP), etc.\ Openings: 1   Summary: With the support of and in partnership with the C3 team, the Senior Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals in improving their safety and quality of life. The Senior Case Manager will provide intensive case management services to their caseload as well as support to the Yellow, Green, and Purple C3teams. The C3 program serves individuals living on the streets of Skid Row in Downtown Los Angeles.The C3 street outreach program is part of a LA County approved strategy to combat homelessness. The goal of C3 is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing, supportive services, and life-saving care. The Senior Case Manager provides harm reduction support and intensive case management for program members, and assists the C3 program staff in trainings and professional growth. The Senior Case Manager will assist the C3 program in being a leader in health-based harm reduction, and trauma informed clinical care for clients. Essential Duties and Responsibilities: 

Train and support Yellow, Green, and Purple team members in utilizing harm reduction and motivational interviewing strategies to support the health and autonomy of clients. Take an active role in agency level trainings/committees on harm reduction, health, and mental health, as well as documentation and time management support. Encourage and promote an environment that is strength based to assist clients in meeting their individual goals. Street outreach to identified homeless persons working with the team to engage in services and accept treatment from the program staff assist with the enrollment process as needed. Provide support to team members on assisting clients in navigating health care systems, transitioning from streets to home, and accessing needed resources in the community, with special attention to increasing harm reduction interventions. Provide referrals to allied critical healthy living services for clients on caseload. Refer target clientele to appropriate services (e.g., health, mental health, interim housing, etc.), and collaborate on care. Document and account for time spent on client contacts and activities in client record in accordance with agency and funder standards. Maintain a current, thorough knowledge of ongoing developments and new techniques in the fields of substance abuse, mental health, and behavioral health community resources and utilize these to provide comprehensive, wrap-around services to clients.

  Qualifications:

High School Diploma, GED or Equivalent Experience working with individuals with severe mental illness Awareness of own areas of strength and weakness and a strong desire to grow in clinical and leadership skills Minimum two (2) years’ experience as a case manager and/or a clinical work in a harm reduction-based setting. Passion for working in a high tolerance Harm Reduction model with clients who have multiple barriers. Strong computer, time management, and writing skills: including proficiency in Microsoft Word, Outlook and Excel Detail oriented with excellent time management and organizational skills. Able to learn quickly and work effectively with a wide range of constituencies.

  Preferred Qualifications:

Minimum of one (1) year experience on C3 multidisciplinary teams strongly preferred Bilingual in English/Spanish Bachelor’s degree Minimum 30 WPM typing speed

  Work Environment:

Daily walking independently throughout Skid Row community. Combination of field (street-based) and office environment. Regularly required to sit, stand, bend, stoop, twist; to engage people on the ground. Occasionally lift or carry up to 35 lbs. Walk and drive to different local sites with or without clients (ex: hospitals, shelters, clinics, housing) Regularly walking distances over uneven surfaces and in encampment areas. Will necessitate working in busy, loud, and chaotic environments Will be exposed to elements like cold, heat, rain, dust, noise and odor, as well as clients with severe medical and hygiene needs.
