Als thuiszorgverantwoordelijke begeleid je het thuiszorgproces van a tot z, coach je een team en verbind je zorgpartners. Een veelzijdige rol met échte impact!

Momenteel hebben we een openstaande vacature voor deze functie in regio Brugge. Het gaat om een voltijdse interimopdracht van half januari tot en met augustus 2027.

Jouw opdracht

  • Je bent vanuit je sleutelrol in de thuiszorg de vertrouwenspersoon voor een veelzijdig doelpubliek: gezinnen, alleenstaanden, ouderen... met uiteenlopende zorgbehoeften (chronische ziekte, beperking, psychische problematiek, kansarmoede, dementie...)
  • Je begeleidt het thuiszorgproces van a tot z: je voert het intakegesprek, bepaalt samen met de zorgvrager de zorgdoelen, stuurt de zorg bij...
  • Je stuurt een team van verzorgenden aan: je coacht de verzorgenden in hun werk, bewaakt de kwaliteit van de zorg en maakt een weekplanning op waarin je rekening houdt met
  • de kwaliteiten en talenten van de medewerkers
  • hun affiniteit met specifieke doelgroepen
  • de geldende arbeidsvoorwaarden en werktijden
  • Je bent de verbindingsfiguur tussen alle betrokken partners in zorg (huisarts, verpleegkundige, sociale dienst...) en neemt regelmatig deel aan multidisciplinair overleg.

Jouw kwaliteiten

  • Een bachelor of master in de gezondheids- of sociale wetenschappen hebben (verpleegkunde, sociaal werk, psychologie, pedagogiek...).
  • Mensen met warme en nabije aandacht benaderen.
  • Belang hechten aan behoefte- en doelgericht werken.
  • Graag zelfstandig werken maar je ook gemakkelijk kunnen invoegen in een team.
  • Graag met individuen én met groepen werken.
  • Leergierig zijn.
  • Je vlot kunnen verplaatsen in een ruime regio.
