Registered Nurse (RN) – IDD Services Location: North Austin, TXSalary: $80,000 annuallySchedule: Monday–Friday | Approximately 8:00 AM - 5:00 PM, with flexibility as neededWork Setting: Approximately 75% office-based / 25% field-based Your Role Join our collaborative nursing team supporting individuals with Intellectual and Developmental Disabilities (IDD) in group homes and host homes. This is primarily a nursing case management role, focused on healthcare coordination, clinical oversight, and delegation rather than hands-on nursing. You'll manage an assigned caseload, provide guidance to direct support staff, and travel throughout the general North Austin area for required monthly home visits. Key Responsibilities:
Conduct annual nursing assessments, quarterly reviews, and monthly visits for individuals on your assigned caseload.
Develop, implement, and monitor individualized healthcare plans, responding to changes in condition as needed.
Schedule and coordinate physician appointments, follow-up care, and other healthcare services.
Communicate with physicians, hospitals, pharmacies, and other healthcare providers to ensure continuity of care.
Monitor medications, physician orders, and healthcare documentation to ensure accuracy and compliance.
Delegate appropriate nursing tasks to non-licensed direct support staff, providing the necessary teaching, training, and ongoing clinical guidance.
Participate in person-centered service planning and advocate for quality healthcare outcomes.
Maintain accurate and timely clinical documentation while meeting weekly documentation expectations.
Travel to residential locations using your personal vehicle (mileage reimbursement provided).
Participate in a rotating after-hours on-call schedule shared between two full-time RNs. Rotation arrangements will be coordinated between the nurses. On-call needs are typically low-volume and primarily involve telephone guidance, with occasional in-person responses.

What You Will Bring
Graduate of an accredited RN program with a current Texas RN license.
Confidence in clinical assessments, telephone triage, and independent nursing decision-making.
HCS/IDD experience preferred but not required.
Ability to effectively delegate nursing tasks and provide clinical training to non-licensed staff.
Current driver's license, reliable personal vehicle, current auto insurance, and willingness to travel throughout the service area.
Current CPR/First Aid certification.
Strong organizational, communication, critical thinking, and documentation skills.
Proficiency with Microsoft Office and Outlook.
Self-motivated, team-oriented, and comfortable managing competing priorities in a fast-paced environment.
